- The license holder sheet completed by typing or handwritten and signed
- Medical certificate for drivers
- Receipt showing payment of the value of the driving license
- Certificate issued by the police to the area where product loss or theft of a driving license and proof of invalidity driving license lost or stolen in the Official Gazette
- Driving license, if it was damaged or expired.
