• Visit the concerned authority.
• Fill the application form with the complete information.

• ID card and voter card
• Birth certificate
• Original and copy of the marriage certificate, if applicable
• Original and copy of the divorce, final and irrevocable , as appropriate
• Original and copy of the death certificate of the deceased husband or wife, if the surviving spouse
• Original and copy of the birth certificates of children less than 14 years old
• Original and copy of the proof of home address
