Romania - Apply for Minor Temporary Passport

Fill the application form with complete information.
The application form along with the necessary documents shall be submitted by the parent(s), legal guardian or authorized person.
Pay the applicable fee.

If a juvenile living in Romania, who have not attained age 14:

minor's original birth certificate
original proof of identity of the parents or parent, legal representative or, where appropriate, the authorized person
original proof of payment of taxes and extra fee for issuing passport
if applicable, original proxy statement or agreement of the other parent in the original child custody judgment became final and irrevocable judgment by the court substituted the consent of the other parent, final and irrevocable or, for requests from the surviving parent or person authorized by him, the death certificate of the other parent
original documents issued by the institution that organizes the competition studies or documents issued or approved by the Romanian administrative authority competent in the field or, where appropriate, documents issued or certified by a medical authority Romanian, showing the period and the state or states where the the scholarships, official competitions or medical treatment
Previous passport of the minor if any)
2 colored photograph (3.5 x 4.5 cm)

If a juvenile living in Romania, who have reached the age of 14:

Original identity card or temporary identity card of the minor. Temporary identity card and identity card model (without surname of the parents) must be accompanied by original birth certificate
agreed statements of parent(s), legal representative (not necessary for married minors under the law).
- If the declaration submitted only one parent, the request must be accompanied by either of the following (original)
  - the judgment of conviction substitution of the minor or the agreement of the other parent, both final and irrevocable
  - the death certificate of the other parent
  - documents issued by the institution that organizes the competition studies or documents issued or approved by an administrative authority competent in the field Romanian or, where appropriate, documents issued or certified by a medical authority Romanian, showing the period and the state or states in which the scholarships, official competitions or medical treatment
Original proof of payment of taxes and extra fee for issuing passport
Previous original passport of the minor
2 colored photograph (3.5 x 4.5 cm)

Must be a Romanian citizen.

Passport cost - 100 (to be paid at CEC Bank units)
Consular fee - 32 (payable in units of CEC Bank or the Treasury)
Surcharge of 100 provided for by Law 248/2005 (to be paid in units of CEC Bank or the Treasury)

The validity of a temporary passport is 12 months.
