This procedure explains various ways to Obtain a copy of Adoption Certificate in Quezon City.
You are a Quezon City citizen. You passed through all phases of the steps involved in adopting a child and obtained the Decree of Adoption. You also performed registration of this adoption in your Local Civil Registrar Office. You or your representatives can apply for a copy of Adoption Certificate at a suitable Census Serbilis Center. Please follow the guide below to do it:
- Prepare the requirements:
- The information about the child prior to the adoption as the complete name of the child; the complete name of the father; the complete maiden name of the mother; the date and place of birth.
- The information about the child after the adoption as the amended name of the child; the complete name of the adopting father, his occupation, religion, and age at the time of the adoption; the complete name of the adopting mother, her occupation, religion, and age at the time of the adoption; the date when adoption decree was issued; the date when adoption decree became final and executory; the name of the court and the judge that issued the decree; the complete name and address of the requesting party; the number of copies needed; and the purpose for the certification
- The information about the child prior to the adoption as the complete name of the child; the complete name of the father; the complete maiden name of the mother; the date and place of birth.
- Apply for the Adoption Certificate
Please visit a suitable PSA Serbilis center Directory and follow officer’s guide to get the application form (white) and your queue number from the Information Marshall. Complete the application form and submit it at the Screener’s Desk. If the accuracy, consistency, and legibility of the application are approved, the screener will stamp your application form with “Okay for Payment”. Now you can pay the applicable fee at the Cashier and get the Official Receipt with a stamp containing the date and time of release.
