Apply Online

  1. To Apply for a Health Card / Permit, the applicant has to make the application online via – link
  2. The applicant has to enter the email address and click the “Continue with Email” or click the “Register here” button.
  3. Enter all the registration details and click the “Register” button.
  4. The applicant will receive the email ID and password from the given email address.
  5. The applicant has to click the “Health card” from the following service list.
  6. Now select all the details of beneficiaries and click the “APPLY FOR INDIVIDUAL HEALTH CERTIFICATE”
  7. Enter all the details in the application form and click the “Next” button.
  8. Select the laboratory schedule from the slot and click the “Confirm” button.
  9. Upload the necessary documents which are mentioned in the “Required Documents” section of this page and click the “SUBMIT APPLICATION”
  10. Click the “VIEW OF ORDER PAYMENT” to make a payment online.
  11. Once completed the applicant can download the acknowledgement slip.
  12. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  13. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  14. Once the process is completed, the applicant has to visit the office.
  15. This entire process can be completed within 30 – 40 or as per the timeline advised by the respective department authority.



Apply In-Person

  1. Here's how you can apply for a Health Permit in Quezon City. First, complete a Health Card Application Form (for individuals) which you can obtain from the Sanitation Division Office and submit it together with all the required documents.
  2. Get your Order of Payment and pay Php194.00 for your Health Card at the City Treasurer's cashier.
  3. Proceed to Windows 1 or 2 and submit for laboratory examination your stool and sputum specimen. The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories.
  4. Collect the results at Window 3.
  5. If this is the first time that you are applying (NEW APPLICATION), you will need to attend a HIV/AIDS seminar. Select a schedule in any of the 5 sessions everyday every: 9:00 AM, 10:00 AM, 11:00 AM, 1:30 PM, 3:00 PM.
  6. Have your photo taken at the Health Department for your Health Card.
  7. Receive your Health Card.


Special service

The local government of Quezon City also process application for health certificates in bulk. Companies can apply for the certificate on behalf of their employees.

Companies can request to:

  • Have their employees' stool and sputum examination done at any accredited laboratories.
  • Have their HIV/AIDS seminar for their employees conducted at their company premises.
  • The City Health Department send the lecturer(s) at an agreed upon schedule.
  • Submit their employees' photos for their Health Certificate. If the photo-taking is not done at the Health Department, the ID provided will be a "white" Health Certificate, and not a self-laminated card. This “White” Health Certificate is good only for one year, while the self-laminated Health Certificate has provisions for 3 years use.

Required DocumentsEdit

  • Completed application form for individual (download link available under 'Documents to Use' section).
  • Official receipt of payment of the Sanitary Fee
  • Inspection report of the Sanitation Inspector to include the following among others:
    • Microbiological Analysis of Water- from water laboratory accredited by the QC Health Department
    • Pest Control Contract of Service / Agreement of Contract Service from licensed pest applicator
    • Masterlist of all employees of the establishment with respective designation/ position
  • Original Health Certificate of all employees of the establishment including that of the Manager(s) and/or Owner for presentation.
  • Photocopy of the Health Certificates-for submission
  • Schematic Diagram / Floor Plan of the establishment
  • Occupancy Permit of the establishment
  • Photocopy of previous Sanitary Permit (for renewal)

  • Applicants below 18 years old:
    • Parental consent
    • Birth certificate/baptismal certificate
    • Valid ID of the parent/guardian giving consent
  • Massagists/ Therapist/Massage Clinic Attendants/Reflexologists, etc:
    • Massagists course
    • Original Birth Certificate issued by PSA
    • Marriage Certificate (if married)
    • 1 pc. 2x2 w/ white background
    • 1x1 w/ collar & sleeves
    • Lab. Result : VD Clearance
    • Official Receipt (P 309.00)
    • Massagists’ Training Certificate
    • Date of training: Every 3rd wk of the month Tuesday to Friday; 1-5PM
  • GRO/FRM/DANCER/ENTERTAINER, etc.:
    • Original Birth Certificate from NSO/ Local Civil Registry
    • Marriage Certificate (if married)
    • 1x1 ID picture
    • Lab. Result : VD Clearance
    • Official Receipt (P 259.00)

ValidityEdit

The Health Certificate issued by the Quezon City Health Department is valid for one (1) year from the date of issuance.

  • Renewal: You must renew your health certificate annually by following the same application process.
  • For food handlers and high-risk workers: Some employers may require more frequent renewal (e.g., every 6 months) based on company policies.
