Quebec - Lost or Stolen Driver License

For QuebecEdit
- Go to an SAAQ service centre to replace a license Plus, or any service outlet for the other licenses, and provide one piece of identification bearing your photo to prove your identity.
- For replacement of a damaged license, you may show that license as proof of your identity, provided you are still recognizable on the photo, and the reference number, name and identification number are still readable.
- If you are unable to provide one of the piece of identity bearing your photo, you must provide two other pieces of identity. Please see below for the required documents.
For Outside QuebecEdit
- You must declare the loss or theft of a driver's license with the appropriate authorities where you are staying: local police or the Canadian embassy or consulate.
- You should therefore request an official certificate bearing the seal or written on the letterhead of the issuing body. This certificate must be written in French or English, or be submitted with an enclosed translation.
- To replace your driver's license, mail or fax the official certificate and a duly signed license replacement request to the SAAQ. Ensure that your request contains the following information:
- the subject of the request;
- the expected length of your stay outside of Qubec;
- the reason for your stay outside of Qubec;
- your last name and first name;
- your date of birth;
- your driver's license number, if you know it;
- your address in Qubec;
- your address outside of the province;
- a telephone number where you can be reached outside of Qubec or where a respondent can be reached in Qubec;
- the mailing address where the SAAQ is to send the driver's license;
- your signature.

- a piece of identification bearing your photo:
- a previous driver's license, other than a taxi driver's license;
- an examination ID card issued by the SAAQ;
- a Canadian passport;
- a Canadian Citizenship Certificate;
- a Permanent Resident Card;
- a Canadian Armed Forces Identity Card;
- a Certificate of Indian Status
- two other pieces of identity:
- for the first one:
- a birth certificate issued by the registrar of civil status (Directeur de l'tat civil) or its equivalent in another Canadian province or territory; or
- a proof of legal presence in Canada; or
- a confirmation of permanent residence
- for the second piece of identity must include a photo and be one of the following:
- a Health Insurance Card;
- an identification card issued by a government agency;
- a foreign passport;
- a driver's license issued by another Canadian province or territory;
- a driver's license issued by another country.
- for the first one:
For Outside QuebecEdit
- Proof of employment on a duly signed and dated official document that includes:
- the reason you are working outside Qubec;
- the expected length of time you will be working outside Qubec; or
- Proof that you are attending an educational institution submitted on a duly signed and dated official document that includes the expected length of time you will be attending this institution.
