Apply In-Person

  1. To apply for a change of name in Quebec, the applicant will need to determine if they are eligible, they will have to submit an application to the Directeur de l'état civil.
  2. The applicant will have to submit a form for preliminary analysis. The applicant can find the form under the “Documents to Use” section of this page. Here is the contact Link

  4. If the applicant is eligible to change their name, the office will send all the documents needed to change their name.
  5. The applicant will have to complete this form by filling in all relevant details.
  6. The applicant will have to make a sworn statement before a person authorized by law.
  7. An applicant who is 14 or 17 years of age will have to inform their father, mother or tutor.
  8. The applicant will have to give a public notice.
  9. Submit the completed application form along with supporting documents at the office.
  10. Once all documents have been submitted to the agent, they will forward them for further processing.
  11. The applicant can check if their change of name has been approved after 30 days, from the Directeur de l'état civil’s website.

  • The applicant applies for a change of name if they are 14 years of age or over and domiciled in Québec for at least one year.
  • The applicant can apply for a change of name for a minor if they are the father, mother or tutor, provided the minor is domiciled in Québec for at least one year.
