Procedure

Apply Online

1. To register in my health patient portal for those above 18 years old through online, visit the following link: online portal
2. Now complete the form with mandatory information and tick the check box ‘I agree with the Terms and Conditions and Privacy Policy’ then click on “Next” button.
3. You receive an SMS with an OTP to the mobile number you added in the application form.
4. In the next page you enter the received OTP then click on “Verify” button.
5. Now the authorities will verify your application and validate the submitted documents.
6. After verification process, an email or SMS with a link will be sent to you. Click on the link to activate your account.
