  • A pre-requisite of this application is a notification of death which is issued by the hospital where the death occurred.

Apply In-Person in Qatar

  1. To obtain a death certificate in Qatar, one has to make an application to the Births and Deaths Record Department at the Ministry of Public Health. The Ministry of Public Health MOPH which is the authority responsible for registering Qatari deaths and issuing of their death certificates .
  2. The first step is to visit a birth and registration office of the MOPH to make the application.
  3. Here the applicant will be issued with appropriate application forms for obtaining the death certificate to fill and submit.
  4. The form is the Death Registration Form which is available at the Birth and Registration Offices or can be downloaded through this link: death registration form
  5. Fill in the required information on the application form and attach all the required documents to the application form and make the submission
  6. These required documents are mentioned in the “Required Documents” section of this page. This include
    • Original Passport / ID of the Deceased.
    • Original ID / Passport of the Informant.
    • Notification of Death issued by the hospital
    • Original Death Certificate Issued at the Country Where the Death Occurred (If it occurred abroad)
  7. The officers of the birth and registration office, will process your application and you will be notified when to collect the death certificate.
  8. The death certificate takes a period of three to five days to process.


Applying from outside Qatar

  1. Visit the nearest Qatari mission or embassy broad to make the application for obtaining a death certificate.

  3. You will be issued with an application form to fill and submit together with the following required documents.
    (i) Original ID of the Deceased.
    (ii) Original Death Certificate Issued at the Country Where the Death Occurred
    (iii) Original ID of the Informant.
  4. The Embassy's consular services section will be able to help process your request to obtain a copy of a death certificate upon payment of relevant fees as advised.
  5. You will be notified when the death certificate is ready for collection.

  • A death certificate is an official document issued by the government, which declares cause of death, location of death, time of death and some other personal information about the deceased.
  • There are several reasons why you may need to obtain a death certificate.
  1. Most often it’s to serve as proof for legal purposes. These reasons may include accessing
    (i) Pension benefits
    (ii) Claiming life insurance
    (iii) Settling estates
    (iv) Getting married (if a widow or widower needs to prove that their previous partner has passed)
    (v)Arranging for a funeral
