Procedure

Apply In-Person:

1. To apply for addition or cancellation of medical service within healthcare facility, applicant has to visit the  Health Facilities Licensing and Accreditation Department at the Ministry of Public Health (MOPH) office.

3. Obtain the application form from the respective officer at MOPH office.
4. Now complete the form with necessary information and make sure you have all the documents in hand that are listed under the “Required Documents” section of this page.
5. After filling the form, attach the documents mentioned in the required documents section below with the completed application form.
6. Check the application form and then submit it to the respective officer in the health facilities licensing and accreditation department in the MOPH office. The address is mentioned in the office locations & contacts section below.
7. Now the authorities will verify your application and validate the submitted documents.
8. After the verification process, medical service within healthcare facility will be added or cancelled as per your request.
