Apply In-Person:
- Application form for registering as a veterinary practitioner in Punjab is available in the Punjab Veterinary Council : Link
- After getting the application form, the applicant has to fill in all the necessary details in the form.
- Applicant should make sure to carry the originals and photocopies of the documents that are listed under the “Required Documents” section of this page.
- Applicant has to attach the photocopies of the documents along with the application form while submitting it at the office and a fee for the registration has to be paid. (Please pay fee as mentioned by the authority)
- The registration certificate will be provided to the applicant within 30 days if the application passes through all the other procedure successfully.

- Three (3) passport size photographs [recently taken].
- Date of Birth (10th class Mark memo or Matriculation or Secondary Examination Certificate) in Original and 2 Xerox Copies.
- Intermediate Education Certificate in Original and 2 Xerox Copies.
- Provisional Degree Certificate (PDC) in original with 2 Xerox copies.
- Provisional Course Completion Certificate in Original with 2 Xerox copies.
- Provisional Registration Certificate in Original. (Internship Training Certificate) (At the time of Registration, you have to submit your Provisional Registration Certificate in the form of Original).
- Degree Certificate in original with 2 Xerox copies.
- Study Certificate of Degree in original with 2 Xerox copies.
- Aadhar Card Xerox Copy – 2.
