Apply In-Person:
- A valid ration cardholder shall approach their respective ration shop of their area Or the Civil Supplies and Consumer Affairs office as applicable.
- Contact Link in the given page, please click on the "Contact Us" menu for contact details page.
- Please visit the respective office and consult with the designated authority. The authority will provide the details for applying. The applicant shall meet the designated authority as advised and collect the application form or write on a plain A4 sheet as advised.
- Please complete the application form. Enclose a detailed covering letter describing the error and the required corrections. Attach the documents as per the “Required Documents” section of this page along with the appropriate payment of fees, if any, as applicable and submit it to the designated receiving authority as advised.
- After receiving the application, the authorities will examine the application and other documents furnished. If all are in place, the application will be accepted and an acknowledgement receipt will be provided. Please keep it safe for future reference.
- ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification from the department.
- This application will be processed further. The concerned authorities will verify the applicant’s details and documents.
- Once the documents are verified, on a scheduled date, a designated authority from the ward/circle will visit the residence of the applicant as per the address mentioned on the application form to inspect and ascertain the facts to approve the request based on the outcome.
- Once the above said steps are completed, the department will approve the changes. Once approved, the ration card will be issued as per applicable norms.
- The entire process can be completed within 2 – 3 weeks' time or as per the timeline notified by the department.
- The status, viz. Approval or Rejection, if any, will be conveyed to the applicant through an “SMS” or as per norms.
- The ration card will be hand-delivered or delivered through post or as per applicable norms.

- Please use the following link to apply online link
- In the given page, please click on the “Change of Name of Member” option to get a new screen.
- Please go through the instructions and click the “Continue / Proceed” button to reach the “Register here - Change of Name Service” screen.
- Here, please select the district from the dropdown list against “District”, Please complete the page prompts as appropriate. Enter the mobile number as appropriate (an “OTP” will be sent), please click the checkbox for “I Agree to provide the above details to avail the Online Services.” Declaration and click the “Send OTP” button.
- An “OTP” will be sent to registered mobile number just provided. Please enter the received “OTP” at the prompted space against “Enter OTP” and click the “Submit” button to reach the “Step 3 of 5” page to apply.
- The applicant shall enter/select the prompted details as appropriate.
- Once all the mandatory details are filled, please click the “Proceed” or the applicable button at the bottom to reach the successive section “Step 4 of 5”.
- Please follow the onscreen information and complete the page requirements to complete the session requirements as appropriate.
- The applicant shall process the page requirement to upload the documents required for this process using the applicable button to upload.
- The applicant shall make necessary online payment if required. Once the mandatory details are completed for “Step 4 of 5”section, the applicant will reach the “Step 5 of 5”
- Here, the page will carry a message claiming the successful submission of the application. Please click on the “Click here to download Receipt” button to download the receipt. Also, please click on the “Click here to download Updated Acknowledgement” option to download the acknowledgement. Please keep it safe for future reference.

- Address proof of the applicant (PAN Card /Driving Licence / Bank Passbook / Electricity Bill / Telephone Bill)
- Identity proof of the applicant (Voter ID / Passport / Aadhaar card / Driving licence / Bank Passbook)
- Copy of ration card and original ration card
- Self-declaration (as applicable)
- Birth certificate as per requirement
- In case of name correction due to spelling mistake, please provide Aadhaar card or voter ID or driving licence with correct name
- In case of name correction due to name change, please provide the copy of gazette with new name printed and if the name change is updated in Aadhaar card or voter ID or driving licence already, then provide these as proof
- For correction in address due to change of street name or road name or any other reason, then provide the respective proof as appropriate.
- For correction in address due to subdivision of land, then please provide the sale deed or respective registered record as applicable
- Aadhaar Card
- If the age of the applicant (for whom the name correction is required) is less than 5 years, then a copy of Aadhaar card is not mandatory. In that case a copy of birth certificate of such applicant is to be submitted for personal details.
- Note:
- Always take original along with copy of document while applying.
- Apart from the above documents, the authorities may ask for additional information or documents. Please provide them for processing.

EligibilityEdit
- A valid ration cardholder is eligible to apply for the correction or update of name or address in their respective valid ration card due to any of the following reasons.
- When any of the person’s name listed in the ration card is misspelled.
- When any of the person listed in the ration card got their name changed by applying for change of name through gazette.
- When the address is not correct in the ration card.
- The address change due to subdivision of the property (new plot number or door number etc).
- The address change due to change of area name or street name or road name announced by the government.

- Usually Family identity card is valid for lifetime and will be renewed in 5 years or based on announcement.

- Please mention the details (name / address) as per the documents that are provided as proof.
- Incomplete application will be rejected.

- For implementation of the Targeted Public Distribution System (TPDS), Ration Card is being issued to all the heads of families, with a complete address and details about the family members included in the card.
- Ration card is essential to apply for passport, PAN card or a domicile certificate or to even get the name registered for electoral rolls to get a Voter ID.
- If the ration card doesn’t carry the name and address correctly, it will be of no use in providing them as a supporting document for any of the government related needs.
- It is must for the beneficiary to get the name or address corrected in the ration card.
