Procedure

Apply In-Person:

1. The applicant will have to visit the Vital Statistics office, find the address under the “Office Locations and Contacts” section of this page. Here is the contact Link.
2. Download and complete the application form, use the following Application Form.
3. Fill in all relevant details such as name, sex, DOB and so on.
4. The applicant will have to carry documents as mentioned under the “Required Documents” section of this page.
5. Submit the completed application at the Vital Statistics office.
6. Make the fee payment, find all fee-related details under the “Fees” section of this page.
7. The applicant shall receive their certificate by mail within 5 business days.

1. To apply for a birth certificate, the applicant will need to visit the Vital Statistics website, use the following Link.
2. The applicant will see an application page, enter all relevant details.
3. Once all the details have been entered click on “Save”.
4. Click on the birth certificate option on the top.
5. Enter all relevant details and click on submit.
6. The applicant will have to make a fee payment, find all fee-related details under the “Fees” section of this page.
7. The applicant shall receive their certificate by mail within 5 business days.

Eligibility

• The applicant should be applying for their own birth certificate.
• The applicant can be a parent of the person who is the subject of the certificate if that person is under 19.
• The applicant should be a custodial guardian if the parent is incapable, and the subject of the birth certificate is under 19.
• The applicants birth should have been registered in Prince Edward Island.

Fees

• The applicant will have to pay fees, for further details use the following Fee.

Validity

• A birth certificate is valid forever and does not need to be renewed.

The applicant can order 2 types of birth certificates:

• The short-form copy of a birth certificate includes;
  • Name of individual
  • Date of birth
  • Place of birth
  • Sex
  • Registration date
  • Registration number, and
  • Date issued.
• The long-form includes;
  • Name of individual
  • Date of birth
  • Place of birth
  • ex
  • Registration date
  • Registration number
  • Date issued, plus
  • The names, and the province or country of birth, of the parents.
