  1. To Replace a Lost or Stolen or Damaged Passport, the applicant has to use the following link
  2. To begin, click the “Schedule an Appointment” button.
  3. Read the Terms and Conditions page. After reading, proceed with the booking process by clicking the “START INDIVIDUAL APPOINTMENT” button.
  4. The appointment page will open, and the applicant will be asked to indicate where they would like to process the application.
  5. Make use of the drop-down menus to select the region and country. For applicants in the Philippines, the applicant doesn’t need to change anything as the default setting is already set locally (DFA sites within the Philippines).
  6. Next, indicate the specific DFA site where the applicant would like to process the application. Use the drop-down menu and scroll through different DFA offices.
  7. After selecting a DFA site, click the “NEXT” button.
  8. Next, the applicant will have to set a date and time for the appointment. If the applicant notices, the calendar is set to show the current month and year. In case the applicant would like to apply for a different month or a different year just click on the month and the year text.
  9. Once the applicant has set a date and time for your appointment, tick the captcha box and then click the “NEXT” button
  10. Complete the Personal Information Form with the complete details.
  11. After finishing filling out the Personal Information Form click the “NEXT” button. Next, the applicant will need to fill out the Family Information Form. Again, once the applicant is done, click the “NEXT” button.
  12. On the next page, the applicant will be asked what type of application the applicant is applying for. select the application type as “Lost” from the drop-down menu.
  13. Complete the rest of the form. The applicant will need to provide the old passport number, the date it was issued and the issuing authority. You will also need to state the basis of Philippine citizenship and designate an emergency contact person. Click "Next" after.
  14. Now, enter the contact information. Click "Next" after.
  15. The applicant will be taken to the Information Review page.
  16. When finish reviewing the application, tick the captcha and consent boxes, and then, click the “CONFIRM” button to finally submit your application to the DFA.
  17. Now, the applicant is done with the first part of the application process which is setting up an appointment with the DFA. DFA will email a copy of your completed form. Print and bring that form with you on the day of your appointment along with all the other required documents.
  18. Follow the process for this procedure as per instructed by the officer.
  19. This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.


Apply In Person:

  1. To Replace a Lost or Stolen or Damaged Passport, the applicant has to approach the DEPARTMENT OF FOREIGN AFFAIRS link
  2. An application form will be provided to the applicant by the concerned officer.
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. Submit all the documents with the completed form to the concerned officer.
  6. The applicant has to pay the prescribed fee as requested by the authority.
  7. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  8. Then the application will be processed further.
  9. Once the process gets completed, the applicant gets a call from the office regarding the application.
  10. Follow the process for this procedure as per instructed by the officer.
  11. This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.

  • Lost Valid ePassport:
    • Police Report in English (Original and photocopy)
    • Affidavit of Loss in English (Original and photocopy)
  • Lost EXPIRED ePassport (it is like you are applying for a NEW passport):
    • Affidavit of Loss in English (Original and photocopy)
  • Mutilated or damaged passport:
    • Affidavit of Explanation
    • The applicant's mutilated/damaged passport
