Apply In Person

  1. To Apply for Student Driver's Permit, the applicant has to approach the LTO Central Office link
  2. The applicant has to book an appointment for registering.
    NOTE: Contact the office to get the appointment.
  3. An application form will be provided to the applicant by the concerned officer. or download it from the following link
  4. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  5. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  6. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  7. Submit all the documents with the completed form to the concerned officer.
  8. The fee for this procedure is mentioned in the “Fees” section of this page.
  9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  10. An acknowledgment receipt will be provided. Please save it for future reference.
  11. Once the process is completed, the applicant will do the photo and signature date base.
  12. Once the process gets completed, the applicant gets a call from the office regarding the application.
  13. Once completed in vision tests, the applicant has to do the biometric procedure in the office.
  14. The applicant has to get the student Driver's License from the office.
  15. This entire process can be completed within 30 days or as per the timeline advised by the respective department authority.

  • Original and photocopy of the applicant's birth certificate, and marriage contract (for married women), duly authenticated by the Philippine Statistics Authority (PSA) or valid passport.
  • Original and photocopy of any issued ID with photo and signature of the applicant.
  • Copy of the applicant's medical certificate. (To be transmitted electronically to LTO by the accredited medical clinic).
  • Copy of medical certificate.
  • Complaint copy (if it is stolen)
  • Affidavit of loss

  • Original and photocopy of passport with the entry date of at least one (1) month and visa six (6) months from date of application, or if born in the Philippines, presentation of the original and submission of one (1) photocopy of the birth certificate duly authenticated by Philippine Statistics Authority (PSA).

Duplicate Student Driver Permit:

  • Certification Fee P 100.00
  • Duplicate Fee P 50.00
  • TOTAL P 150.00

  • The validity for the student driver permit is one year.
