Procedure

• Philippine laws require food and non-food establishments to apply for a permit and pass a sanitary inspection. A local health unit in every city hall oversees the issuance of hygiene certifications.

Apply in Person

1. Business establishments issued with a Sanitary Permit or Certificate by the City Hall/municipality, Health, and Sanitary Department will be subjected to re-inspected quarterly by the Sanitary Inspectors.
2. Should there be any complaints/violations or the establishment poses health hazards to the nearby residents, the Division will issue three (3) notices of violation to comply with therequirements or conditions.
3. If the Division didn’t get any response or if requirements/conditions have not been complied with, the establishment will be issued a Temporary Closure Order and the Sanitary Permit may be revoked.
4. Where the business establishment has complied with all the health standards the holder of the Sanitary Permit or Certificate is eligible to apply for renewal of the permit.
5. Application for renewal of the permit should be submitted to theCity Hall/municipality, Health, and Sanitary Department, where the business is to be locateda few days prior to the expiration date of the current permit.
6. Applicants are issued a Sanitary Permit Application form (Initial/Renewal) to complete, once completed please don’t forget to attest (Sign) the form.
7. The application will also be accompanied by sanitary permit renewal fees as may be prescribed by the Authority.
8. Upon a successful document evaluation phase has been completed, aSanitary Permit is issued to the applicant.

• A business establishment with a sanitary permit that has not yet been cancelled, suspended, or revoked by the issuing Authority.

Validity

• Validity of the sanitary permit shall be mentioned on the document and shall remain valid for that stipulated time unless suspended, evoked, or cancelled by the Authority.
