Procedure

• Philippine laws require food and non-food establishments to apply for a permit and pass a sanitary inspection. A local health unit in every city hall oversees the issuance of hygiene certifications.

Apply in Person:

1. To apply for Sanitary Permit or Certificate applicant should proceed to the City Hall/municipality, Health, and Sanitary Department, where the business is to be located.
2. Here the applicant is issued the Sanitary Permit Application form that contains the following section to be completed. Once completed please don’t forget to attest the form.
   • Date of Application
   • Transaction Number
   • Name of Establishment
   • Address of the Establishment
   • Name of the Owner
   • Designated Representative
3. Attach the supporting documentation as listed under the” Required Documents” section of this page and should be submitted for verification.
4. Upon receipt of the application dossier, the applicant is issued with a Temporary Sanitary Permit which is a requirement when applying for a Business permit, the actual Sanitary permit may only be issued after the completion of mandatory company office inspections and/or health tests for employees.
5. The Sanitary Inspector will conduct an inspection once he receives the Mission Order. He will conduct the following:
   • Water Analysis Results. The test will be conducted by recognized water laboratories and testing centers. The analysis shows that the water is free from any microbiological contamination that may be hazardous to everybody’s health.
   • Pest Control. Every city and selected municipality has its own accredited pest control operators.
   • Health certificate for all employees. All employees and workers of the company must undergo X-ray and drug testing to ensure that the workers are fit to work.
6. During the inspection, if any modification or upgrading is required, the Corrective Action and Preventive Action Report (CAPA) report will be issued and the client will be required to rectify the problem.
7. Upon successful implementation of the CAPA, a follow-up inspection may be conducted by the health inspector to ascertain the effectiveness of the implementation (where applicable).
8. Applicants who meet the Health Standards requirements will be issued the sanitary permit, which shall mention holder particulars and validity period.

Note:

• Business establishments are re-inspected quarterly by the Sanitary Inspectors
• Should there be any complaints/violations  or the establishment poses health hazards to the nearby residents, the Division will issue three (3) notices of violation to comply with the requirements/conditions
• (If the Division didn’t get any response or if requirements/conditions have not been complied with, the establishment will be issued a Temporary Closure Order and the Sanitary Permit may be revoked.

• Fees varies from one city hall/municipality office to another, please confirm with the City Hall/municipality, Health, and Sanitary Department, where the business is to be located.

Validity

• Validity of the sanitary permit shall be mentioned on the document and shall remain valid for that stipulated time unless suspended, evoked, or cancelled by the Authority.
