Apply In Person

  1. To Apply for Radio Type Approval, the applicant has to approach the Philippine Drug Enforcement Agency link
  2. An application form will be provided to the applicant by the concerned officer.
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach photocopies of the required documents along with the application form while submitting it.
  6. Submit all the documents with the completed form to the concerned officer.
  7. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  8. The applicant has to pay the prescribed fee as requested by the authority.
  9. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  10. Once the application has been approved by the PDEA, the applicant will need to take the proficiency test.
  11. The proficiency test will assess the knowledge and skills in the field of drug testing. The applicant can check the PDEA website for the schedule of the proficiency test.
  12. If the applicant passes the proficiency test, the applicant will receive the drug testing laboratory license from the PDEA. The applicant can now legally operate a drug testing laboratory in the Philippines.
  13. This entire process can be completed within 15 - 30 days or as per the timeline advised by the respective department authority.

  • A copy of the blueprint and floor plan of the laboratory
  • A copy of the organization chart and job descriptions of laboratory personnel
  • A copy of the Standard Operating Procedures (SOPs) for the laboratory
  • A copy of the Quality Manual for the laboratory
  • A copy of the Calibration and Maintenance Schedule of laboratory equipment
  • A copy of the Certification of Good Laboratory Practice (GLP)
  • A copy of the biosafety certificate (if applicable)
  • A copy of the environmental permit (if applicable)
  • A copy of the accreditation certificate from an accrediting body (if applicable)
