Apply Online

  1. To Apply for a Copy of a Certificate of Live Birth (COLB), the applicant has to make the application online via link
  2. The applicant has to click the “Order Now” button.
  3. Select the birth option and click the terms and conditions.
  4. Followed by clicking the “Continue” button.
  5. Select the option and click the “Continue” button.
  6. Enter all the details in the application form and click “Continue”
  7. And upload all the necessary documents that are mentioned in the “Required Documents” section of this page and click “Continue”
  8. Pay the prescribed amount fee in online mode.
  9. Once completed with all the details click the “Submit” button.
  10. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  11. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  12. Once the process gets completed, the applicant gets a call from the office regarding the application.
  13. The applicant can download the copy of the certificate.
  14. This entire process can be completed within a week or as per the timeline advised by the respective department authority.



Apply In Person

  1. To Apply for a Copy of a Certificate of Live Birth (COLB), the applicant has to approach the PHILIPPINE STATISTICS AUTHORITY link
  2. An application form will be provided to the applicant by the concerned officer. or it can be downloaded from the following link link
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the concerned officer.
  7. The applicant has to pay the prescribed fee as requested by the authority.
  8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  9. Then the application will be processed further.
  10. The officer will review the application and may request additional information or clarification if necessary.
  11. The applicant will receive a printed copy of the COLB. If requested delivery, it will be sent to the specified address.
  12. After receiving the COLB, carefully review it to ensure all the information is correct. If one find any errors, contact the PSA immediately to request corrections.
  13. This entire process can be completed within a week or as per the timeline advised by the respective department authority.

The following are eligible to apply for a copy of a Certificate of Live Birth (COLB) in the Philippines:

  • The person named in the COLB
  • The spouse of the person named in the COLB
  • The parents of the person named in the COLB
  • The direct descendants of the person named in the COLB
  • The guardian of the person named in the COLB, if the person is a minor
  • The court or proper public official, whenever absolutely necessary in administrative, judicial or other official proceedings to determine the identity of the child's parents or other circumstances surrounding his birth
  • The nearest relative or kin of the deceased person, in case of a death certificate
