Apply In Person

  1. To Apply for Accreditation of Civil Society Organizations, the applicant has to approach the Department of Agriculture link
  2. An application form will be provided to the applicant by the concerned officer. or it can be downloaded from the following link
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the concerned officer.
  7. The applicant has to pay the prescribed fee as requested by the authority.
  8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  9. An acknowledgment receipt will be provided. Please save it for future reference.
  10. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  11. Once the process gets completed, the applicant gets a call from the office regarding the application.
  12. This entire process can be completed within 10 - 15days or as per the timeline advised by the respective department authority.

  • Accomplished data sheet with organizational setup
  • Certificate of good standing from Local Chief Executives or the Head of a local religious
  • organization where the program /project will be implemented (authenticated)
  • Valid Mayor’s permit and BIR registration (authenticated)
  • Certificate of registration and/or Certificate of filing from SEC, CDA or DOLE-BRW
  • (authenticated)
  • Certificate of good standing from government agencies (authenticated)
  • Omnibus Sworn Statement (form attached)
  • Copy of bio-data sheet with a recent photo of all officers and articles of Incorporation/Cooperation latest amendment by law, showing the original incorporators/organizers andthe Secretary’s certificate for incumbent officers
  • CSO which has been in operation for less than (3) years, report of accomplishment orany equivalent proof certified by the President and Secretary that it had previouslyimplemented similar projects shall be required, Sunset provision of GPPB 17-2016
  • Disclosure by the CSO Directors and its Trustees of other related business, if any, andextend of ownership therein
  • Authorization letter to apply for accreditation (if with mother or umbrella NGO/CSO)
  • Affidavit of Secretary of CSO that none of its incorporators, organizers, directors, or official is an implementing agency who are authorized to process accreditationapplication
  • Certificate of registration with CDA (authenticated)
  • Certificate of compliance issued by CDA

Need for the DocumentEdit

  • Accreditation- the process of assessing the applicant to determine whether a Civil Society Organization (CSO) is eligible to implement programs and/or projects using DSWD funds after meeting all the set criteria and requirements. A CSO Certificate of Accreditation shall be issued as proof of official recognition.
