- State issued drivers license or non-driver photo ID (if address has been changed, include copy of update card).
- If you do not have acceptable photo identification, an eligible requestor possessing government issued photo ID may apply. If an eligible requestor is unable to apply, you may submit a completed Statement from Requestors Not Possessing Acceptable Government-Issued Photo ID with two documents verifying your current address.

EligibilityEdit
Only certain people can request a fetal death certificate. You must be:
- Immediate family member
- Extended family member who indicates a direct relationship to the stillborn fetus

- The Division of Vital Records issues only certified copies of fetal death records with the raised seal.
- There are 3 ways to obtain a certified copy of a fetal death certificate in person, by mail, by online.
- Be sure to complete all required information on the application form.
- There is a $9 fee for each certified copy. This fee may be waived for the dependent of an individual who served or is currently serving in the Armed Forces.
