Apply In-Person

  1. To apply for a change of name in Ontario, the applicant will need to visit the nearest Service Ontario centre, here is the contact Link
  2. The applicant will be given a form to fill out and make sure all the details are filled in, correctly. The applicant will receive different forms depending on if they are an adult, child, or want to change their name to a single name.
  3. Here is the link to the application forms that are in the "Documents to Use" section of this page.
  4. Here are the links to the application form Adult name change application or Child name change form or Reclaiming indigenous name fee, there is no fee that needs to be paid, so fill this form (To reclaim a name use the adult or child form).
  5. Carry all supporting documents. The applicant can get further information from the registry agent or check under the "Documents to Use" section of this page.
  6. Submit the completed application form along with supporting documents to the office.
  7. Once all documents have been submitted to the agent, they will forward them for further processing.
  8. Once submitted and approved, the applicant will receive a certificate with their changed name within 6–8 weeks.

EligibilityEdit

  • The applicant should have lived in Ontario for 12 months.
  • The applicant must remain in Ontario until the change of name document has been approved.
  • The applicant must be 18 years of age or older.
  • If the applicant is under 18 years, they can apply under certain conditions, and these are:
    • If the applicant is changing the name of a minor but the minor turns 18 before the application is processed, in which case, a new application will need to be submitted.
    • The applicant needs to make sure there is enough time to process the application before submitting an application to a registry agent.


FeesEdit

  • The applicant will have to pay a fee of 137$ for an adult change of name.
  • For changing the name of a child, the applicant will either have to pay 122$ or pay 22$ if the form is being submitted along with the parent’s change of name form. The same applies to a single name change.
  • If the applicant is reclaiming their indigenous name, they can do so free of cost.

  • The document is valid forever and does not need to be renewed.
