• Complete the required form
  • BMV1173 Record Request
• Pay the fees

By Mail

• Send a completed BMV 1173 Record Request with fees attached to

Apply Online

• Go to the BMV Site.
• Access the free online driver abstract system

• Social security number
• Drivers license
