Procedure

Apply In-Person:

1. To obtain a Certificate of Good Health in person, the applicant has to visit the Nigeria Centre for Disease Control.
2. The address and contact details can be found in the following contact link
3. Visit the office and obtain an application form to get a Yellow Fever Vaccine, from the relevant department.
4. Make sure that you have all the documents listed in the “Required Documents” section of this page.
5. Complete the application form as required and submit it to the relevant official at the office.
6. Health Care workers over there will proceed to make arrangements for giving you a Yellow Fever Vaccine.
7. Once you are jabbed, you will be asked to wait for some time and they will proceed to process your certificate.
8. After the processes are over, the applicant will be issued with a Yellow Fever Certificate as a proof of vaccine.

Any traveller in Nigeria, who is above 9 months old or above, should get a vaccination.
