Procedure

Apply In-Person:

1. To apply for Post Mortem of Birds in person, the applicant has to visit the Central Diagnostic Laboratory or any nearest branch of the National Veterinary Research Institute.

3. Visit the CDL and approach the relevant authority to apply. Obtain a relevant application form from the authority.
4. Make sure that you have all the documents listed in the “Required Documents” section of this page.
5. Complete the application form with appropriate information under relevant sections. Attest it with your signature once completed.
6. Once completed filling the application form, make sure that you have attached all the documents required.
7. Then, submit the application package in person to the relevant authority at the office, along with the dead body of the bird. Make the stipulated payment as directed by the officials.
8. After submission, the application and other documents will be forwarded for examination and verification processes.
9. Once the verification and validation processes are over, the application will be approved and the officials will proceed to take required steps for the Post Mortem.
10. When the Post Mortem is over, the report will be issued to the applicant after proper intimation via phone or email.

Instructions

• The applicants should bring at least two weak birds from the same flock of the dead bird, in order to help the veterinarians in investigation.
• The applicants should bring the dead birds preserved in cold storage, in case if they are from a long distance.
