Apply In-Person:

  1. To apply for Community Based Social Health Insurance Scheme in person, the applicant must visit the office of National Health Insurance Scheme of Nigeria.

  3. Visit the office and obtain an application form for Community Based Social Health Insurance Scheme from the relevant department.
  4. Complete the application form with appropriate information under relevant sections. Attest it with your signature once completed.
  5. Once completed filling the application form, make sure that you have attached all the documents listed in the “Required Documents” section of this page.
  6. Then, submit it in person to the relevant authority at the office. Make the stipulated payment as directed by the officials.
  7. After submission, the application and other documents will be forwarded for examination and verification processes.
  8. Once the verification and validation processes are over, the application will be approved and the officials will proceed to take required steps for registration.
  9. Then, the applicant will be informed through registered email address or mobile number about the further tasks to be accomplished.

  1. To apply for Community Based Social Health Insurance Scheme online, the applicant has to visit the online registration portal of the National Health Insurance Scheme of Nigeria by clicking the following Registration link
  2. Locate the registration option as applicable to your requirement and category of application; make a click on it to start the registration process.
  3. Follow the on-screen instructions, provide the necessary details and complete the registration process. After registration, proceed to select the application option as applicable.
  4. Complete the online application by following the on-screen instructions and providing the details asked for.
  5. Upload the documents as required and also make the payment as applicable by clicking the appropriate buttons and options.
  6. After completing the application and making the payment, submit the application by clicking Submit button.
  7. After submission, the application and other documents will be forwarded for examination and verification processes.
  8. Once the verification and validation processes are over, the application will be approved and the officials will proceed to take required steps for registration.

  • Community Based Social Health Insurance Programme is a non-profit making programme for a cohesive group of households/individuals or occupation-based groups.
  • Persons wishing to get benefited on the basis of mutual aid within the members of the programme and the collective pooling of health risks, can make use of this programme.

  • This shall be voluntary and open to all residents (families) of the participating communities/occupation-based groups (including retirees). The family or individual members shall be the unit of registration.
  • In order to achieve a critical pool of funds to ensure financial viability, as well as to address the problem of adverse selection, communities/occupation-based groups shall have at least 50% of members willing to participate (or a minimum of 1000 members).
