Procedure

1. If you have lost your mobility parking permit, you must get in touch with the nearest branch of CCS Disability Action to let them know that it was lost so they can cancel the permit and issue you a new one.
2. To apply for a replacement you will need to fill out an application form again. A link to download the form is provided under 'Documents to Use' section. Note: You won't need a doctor to assess you again as they have your details already.
3. Pay the replacement cost of $35.00.
