Procedure

1. To apply for a Community Services Card you can request for one by calling Work and Income - 0800 999 999.
2. You may also pick up an application pick up a form from:
   1. your doctor
   2. a pharmacy, or
   3. a Work and Income service centre, or
3. You may also download a form from the Work and Income website - link
4. Work and Income will send you a letter telling you if you'll get a card. If you qualify, your card will be included with the letter.
   It can take around 3 weeks for your card to arrive. Keep receipts for doctor's visits or prescription costs and you can claim them back later.

Eligibility

You can apply for a Community Services Card if you are:

• at least 18 years old (or at least 16 and in full-time tertiary study), and
• on a low to middle income (the amount depends on your family situation), and
• a New Zealand citizen or permanent resident living here.

You can also apply if you have (or have applied for) refugee or protection status.

Fees
