You should complete the form yourself, if you are able to do so
- When applying for a New Zealand passport, Certificate of Identity Travel Document or Refugee Travel Document, you should fill in the form yourself, using your own handwriting.
- For child passport applications, a parent or legal guardian should complete the form on their childs behalf. A parent must sign the consent to issue a child passport.
If you cannot fill in the form yourself
- If you cannot fill in this form yourself because you have a disability or language difficulty, someone else can fill in the form for you. This person must complete the section Statutory Declaration. You must then sign your own form in the section Your Applicant Declaration, if you are able to do so.

Payment Methods
- EFTPOS or cash payments can only be made in-person at a Passport office.
- Cheques and money orders
- Fill in all parts of the cheque or money order.
- Make it payable to the 'Department of Internal Affairs'.
- Credit cards
- Provide all of the details requested in the payment section of the application form.
- New Zealand Passport office accept VISA, MasterCard, Visa debit cards and American Express.
- DO NOT SEND CASH IN THE POST.
ValidityEdit
The New Zealand Passport is valid for a maximum period of 5 years. The validity period starts from the date of issue and runs until the date of expiry.

Processing TimeEdit
Is your child's travel urgent?
- A standard application for a passport takes up to 10 working days to process. If your child needs to travel urgently or because of an emergency situation, for an additional fee New Zealand Passport office can usually process their passport application in up to 3 working days.
- If your child needs to travel because of serious illness or the death of a family member, you may not have to pay this extra fee, or it may be refunded after your childs passport has been issued.
Standard service: up to 10 working days
Urgent service: up to 3 working days
Callout service: There is an extra fee for using the Callout service. This service is available by appointment outside business hours, 7 days a week, for emergency situations.

- All children (under 16 years) and babies need to travel on their own passport
- Consent from a parent or legal guardian is required before New Zealand Passport office issue a childs passport. Read this page carefully to make sure you are filling in the correct form.
- Is your child under 16 years old?
- Are they applying for their New Zealand Passport for the first time or renewing their New Zealand passport?
- Have they changed their name since their last passport was issued?
- Has their most recent passport been lost or stolen?
- While completing the application form you need
- Two (2) identical recent colour photos of the child
- Someone to act as a witness who is over 16 years of age and has a valid New Zealand Passport
- The correct application fee
- Sometimes New Zealand Passport office may ask you to send original documents. In most cases, New Zealand Passport office only need original documents if they have been issued outside New Zealand.
Instruction to complete the forms
- Use a BLACK or BLUE ballpoint pen.
- Write in CAPITAL letters inside each box
- Put a line through mistakes. DO NOT use correction fluid or correction paper.
Your child's previous passport
- If your child has a New Zealand passport you are not required to send it to New Zealand Passport office. Once your child's application has been received your child's previous New Zealand passport will be cancelled and cannot be used for travel.
Childs witness
- The childs witness must fill in this section in their own handwriting and write the childs name on the back of one photo, and sign and date the back of the photo.
- The childs witness must write in CAPITALS and cross out any mistakes.
Who can be the childs witness?
- The childs witness must:
- have a valid New Zealand Passport
- have known the child for 1 year or more, or since birth for a child under 1 year old
- be 16 years of age or older
- not be related to the child or part of the childs extended family group
- not live at the same address as the child.
- The childs witness needs to provide their passport number, name, date of birth and contact details, including a day-time phone number.
