- Print and complete the application form, then mail to the Registry or deliver them to a Registry office in person along with your identification.

Required DocumentsEdit
- You must establish proof of your current name and all other names ever used with evidence such as: marriage certificates, divorce papers, deed poll documents, change of name certificates, etc.
- Photo identification must be provided. If you have not supplied official photo identification such as a passport or driver licence you must provide one passport sized photo and a guarantor must complete the section on page 5 of the statutory declaration.
- One form of identification must confirm your current residential address
- All documents must be current and additional identification or information may be requested.
- Please provide the following document
- New South Wales birth certificate
- You should also provide at least one of the following documents
- Current Australian drivers licence
- Current Australian passport or foreign passport
- Current NSW Roads and Maritime Services ID card
- Current Firearms licence
- And you should also provide at least one of the following documents
- Medicare card showing applicants name
- Current credit/debit card or ATM card or bank passbook in applicants name
- Current Centrelink card in applicants name
- Current Australian Department of Veterans Affairs card
- Current Australian security guard licence or crowd control licence
- Current Australian tertiary education institution ID card

EligibilityEdit
- You are eligible to have your name changed in NSW if:
- Your birth is registered in NSW or
- You were born overseas and have been resident in NSW for three (3) consecutive years immediately prior to your application or
- Your birth is not registered in NSW and a protection order has been made to protect you and/or your children from domestic violence.
Persons restricted from changing their name in NSW
- The Births, Deaths and Marriages Registration Act 1995 restricts certain classes of persons from changing their name without the permission of a supervising authority. It also restricts people from making a change of name on behalf of a restricted person. Restricted persons are:
- An inmate of a correctional centre.
- A person on remand.
- A parolee.
- A periodic detainee.
- A person who is subject to a supervision order.
- A forensic patient.
- A correctional patient.
Exemptions to these restrictions are:
- Persons who have been convicted of an offence under a law of the Commonwealth.
- Persons who are the subject of a warrant under the Commonwealth Defence Force Discipline Act 1982 and are in a correctional centre pursuant to a punishment imposed under that Act.
- Persons who are detainees within the meaning of the Commonwealth Migration Act 1958 and who are held in a correctional centre.
- Persons who are in the keeping of a correctional officer under section 250 of the Crimes (Administration of Sentences Act) 1999.
- Persons who are a former serious offender because of the commission or alleged commission of an offence under a law of the Commonwealth.

InstructionsEdit
- The Registry records official changes of name for adults and children (under 18 years).
- Only eligible person can apply for name change.
- The Land Titles Office no longer registers Deed Polls or Instruments Evidencing Change of Name.
Certificates
- Change of Name includes registration fee and either a Birth or Change of Name Certificate.
- You must return all of your original certificates with your application including any NSW Birth or NSW Change of Name Certificates and extracts.
Adult Change of Name
- If you were born in NSW
- You receive a Birth Certificate or a Change of Name Certificate.
- To purchase both, please add $42.00 to your fee.
- Your birth registration will be amended and will include details of any previous names used.
- If born overseas
- You receive a NSW Change of Name Certificate
Child Change of Name
- If your child was born in NSW
- You receive a Birth Certificate or a Change of Name Certificate.
- To purchase both, please add $42.00 to your fee.
- The child's birth registration will be amended to include change of name. Parents' names are not changed.
- If child born overseas
- You receive a NSW Change of Name Certificate
- If a person alters their name or uses an additional or other name with the intention to act fraudulently or with an intention to deceive or in any other way that contravenes the law, they may be subject to criminal proceedings.

- To organise a Change of Name interview, please phone 1300 655 236 to arrange an appointment. Alternatively, the Registry accepts applications by mail.
