New South Wales - Obtain Proof of Identity (or) Photo ID Card

- Complete the Photo Card application form (Roads and Maritime Services (replacing Roads and Traffic Authority) form 1359) which is available at all motor registry and return the completed form to any motor registry with appropriate proof of identity documents, proof of residential address and the administration fee.
- To ensure the card's legitimacy, you must pass a strict proof of identity process.
- Produce all required original documents to prove who you are to Roads and Maritime Services (replacing Roads and Traffic Authority). Your current residential address and signature must appear on at least one of these documents.

Proof of identity for individuals
- Protecting your identity is important for you and for Roads and Maritime Services. Because a driver licence is used by many groups as a reliable form of identification, Roads and Maritime Services has to make sure the integrity of licences is protected.
- Identity fraud is an area of significant criminal activity. If you are a victim of identity theft, your licence or credit card could be used unlawfully and traffic offences and demerit points could be incorrectly assigned to you.
- Roads and Maritime Services needs to see documents that identify you and may ask for additional information to verify a document with the issuing authority. In a small number of cases these checks may take a number of days. You will be advised as soon as enquiries are completed.
- Roads and Maritime Services has worked with NSW Police and other relevant groups on proof of identity arrangements. While the arrangements may seem strict, they are necessary to protect your identity and privacy, as well as the driver licence system.
- To prove who you are to Roads and Maritime Services, you need to provide either a NSW-issued driver photo licence or NSW Photo Card that is current or expired within the last two years (provided it is not recorded as lost, stolen or destroyed), or two other documents - one from List 1 and one from List 2.
List 1 POI documents are:
- An Australian full birth certificate showing parental details, or a current photo birth card issued by the NSW Registry of Births, Deaths and Marriages.
- Note: Commemorative certificates are not acceptable (except for 1987 and 1988 NSW Historic Birth Certificates). See the Births, Deaths and Marriages website for information.
- An overseas birth certificate showing parental details provided a passport or an official Australian travel document is also shown.
- A current Australian passport or one that expired within the last two years.
- The following travel documents issued by the Department of Foreign Affairs and Trade: ?A current Certificate of Identity
- A current Document of Identity
- A current Titre de Voyage (must be accompanied by a visa)
- A current overseas passport.
- An Australian naturalisation or citizenship document issued by the Department of Immigration and Citizenship.
- The following documents issued by the Department of Immigration and Citizenship: ?A Visa Evidence Card (must be accompanied by a current visa)
- A Document for Travel to Australia (up to five years from the date of issue on the accompanying visa)
- A Certificate of Evidence of Residence Status
- A Roads and Maritime Services or Roads and Traffic Authority-issued NSW photo driver licence or NSW Photo Card that has been expired more than two years but less than five years. The licence must display a card number and not have been revoked or be reported as lost, stolen or destroyed.
- A current photo driver licence from another Australian State or Territory or one that expired within the last two years. If your interstate licence shows your middle name(s) as initial(s), you will require another supporting List 1 or List 2 document showing your full name.
- A current Roads and Maritime Services or Roads and Traffic Authority-issued NSW photo Firearm, Security Industry or Commercial Agents and Private Inquiry Agents operator licence.
- A current photo identity card for the NSW Police Force or the Australian Defence Force, excluding civilian staff or family.
- A current consular photo identity card issued by the Department of Foreign Affairs and Trade.
Important: Your List 1 document must show your full name, and the day, month and year of your birth.
List 2 POI documents are:
- A current Medicare card, Pensioner Concession Card, Department of Veterans' Affairs entitlement card or any other current entitlement card issued by the Commonwealth Government.
- A current plastic credit card or account card issued by a bank, building society credit union, *American Express or Diners Club International, showing your name and signature.
- A passbook or account statement up to one year old issued by a bank, building society, credit union, American Express or Diners Club International. An account statement, must show your name, residential or mailing address and account number as well as the financial institution's letterhead or stamp.
- A telephone (landline only), gas or electricity bill up to one year old.
- A water rates, council rates or land valuation notice up to one year old.
- A current student identity card displaying the student's photograph issued by an Australian secondary school, TAFE or university. Where a student identity card has an issue date but no expiry date, it may be accepted up to two years from the date of issue.
- Evidence of enrolment at an Australian secondary school, TAFE or university up to one year old showing the student's name and address and on the institution's letterhead.
- A current plastic 'licence-style' Mobility Parking Scheme (MPS) permit, with or without a photo.
Important: Your List 2 document must show at least your family name and your first given name in full.

- To be eligible for a Photo Card you must:
- Be a NSW resident.
- Be aged 16 years and over.
- Not hold a current driver licence or Proof of Age Card (if you do you will need to surrender your driver licence or Proof of Age Card when you apply for your Photo Card).
- Meet Roads and Maritime Services' (replacing Roads and Traffic Authority) standard proof of identity criteria.

- The Photo Card application form (Roads and Maritime Services (replacing Roads and Traffic Authority) form 1359) is available from any motor registry. Simply return the completed form to any motor registry with appropriate proof of identity documents, proof of residential address and the administration fee.
- Regulation changes were introduced on 30 April 2010 to allow the Photo Card to be issued or replaced free of charge to eligible pensioners, including war widows.
- To ensure the card's legitimacy, applicants must pass a strict proof of identity process.
- You will be required to produce original documents that prove who you are to Roads and Maritime Services (replacing Roads and Traffic Authority). Your current residential address and signature must appear on at least one of these documents.
Keep your card close
- Just like a driver licence, your Photo Card holds important personal information about you so be careful who you give your card details to. Only police officers or Roads and Maritime Services (replacing Roads and Traffic Authority)-authorised personnel are permitted to seize a card. You should not let any person or business scan your card. Businesses may only take a copy of your card with your permission.
- If your Photo Card is lost, stolen, damaged or destroyed, you must notify Roads and Maritime Services (replacing Roads and Traffic Authority) in person at any motor registry as soon as possible. You will need to pay a replacement card fee unless you are an eligible pensioner.
Proof of Age Card
- The Proof of Age card has been replaced by the NSW Photo Card. New and replacement Proof of Age cards are no longer available. If your Proof of Age Card is lost, stolen or destroyed you can apply for a NSW Photo Card.
- From 14 December 2008, Proof of Age cards are no longer valid.
