Procedure

Apply In-Person

1. Following are procedures on how you can apply in-person and change the personal information on your Medicare card. You will need to apply for this procedure if any of your personal information changes, due to you moving to a new address, birth, adoption, death, marriage, divorce, or legal separation.
2. You can request for your personal information to be changed or updated by completing a Medicare Change Request Form. A link is provided under 'Documents to Use' section which you can click to download the form.
3. Print and complete the change request form. Make sure that you fill up the form correctly. Make sure to include in the form all household members affected by the move. Failure to notify New Brunswick Medicare of your address change may result in termination of your Medicare coverage.
4. Submit the completed form together with your proof of identity and residency in person to a Service New Brunswick office.
5. You will be notified by Medicare when your card will be available.

Apply By Mail

1. You can request for your personal information to be changed or updated by mail. In order to do this you will need to complete a Medicare Change Request Form. A link is provided under 'Documents to Use' section which you can click to download the form.
2. Print and complete the change request form. Make sure that you fill up the form correctly. Make sure to include in the form all household members affected by the move. Failure to notify New Brunswick Medicare of your address change may result in termination of your Medicare coverage.
3. Completed forms must be mailed to Medicare directly together with photocopies of your proof of identity and residency. Send your request to P.O.Box 5100, Fredericton, N.B. E3B 5G8.
4. You will be notified by Medicare when your card will be available.

• Indian Status Card
• Baptismal certificate (if place of birth is identified)

Non-Canadians - mandatory:

• Canadian Immigration Identification Records and Entry Stamp on their Passport
• Proof of enrollment from an approved learning institution

• Valid New Brunswick Driver’s License
• Valid Passport (Canadian or foreign)
• Baptismal Certificate (if place and date of birth indicated)
• Valid Permanent Resident Card (front & back) or Record of Landing Document example: (Form # IMM 1000)
• Active Temporary Resident Permit issued by Citizenship and
• Immigration Canada
• Certificate of Canadian Citizenship
• Canadian Indian Status Card (front & back)
• Legal Name Change Document
• Student or Employee ID Card
• Previous Provincial Health Card

Documents that may be accepted as proof of residency

• Mortgage Document
• Rental or Lease Agreement (must be signed by landlord & tenant)
• Utility Bill - not older than 2 months (phone, energy, cable/satellite, water/sewer)
• Employment Confirmation - current (pay stub or letter from employer on company letterhead)
• Insurance Policy (home, tenant, auto)
• Valid NB Motor Vehicle Registration
• Valid NB Driver's License
• Child Tax Benefit Statement
• Property Tax Bill (current year)

Eligibility

You may apply for the procedure if you are holding a valid New Brunswick Medicare card.

Fees

Explain the fees structure which is required for obtaining the certificate/document.

Validity

• Cards are valid for five years.

• Your card is valid until the last day of the month indicated. For example, a card showing the date "03/2020" is valid until March 31, 2020.

• Provided you are eligible for coverage and have kept Medicare informed of your current address and/or residency information, you may receive your card approximately 1 month before this date. Destroy your old card and use the updated card effective immediately.
