• Apply for a Nebraska State ID Card, and present documentation of:
  • Identity containing Name and Date of Birth.
  • Principal Address in Nebraska (at least two documents are required).
• Pay the required fees.

To apply for an ID card, you'll need to provide one form of acceptable identification that includes both your full legal name and date of birth, and two documents verifying your address. In some cases, you may be asked for proof of your Social Security number, too. If you're younger than 18 and can't provide proof of your address, bring two acceptable documents showing the address of a parent or guardian.

Instructions

Any person who is a resident of the state of Nebraska may obtain a State Identification (ID) card with a color photograph of the person included.

One form of identification is required to obtain an ID card. This forms of identification must be one primary document from the Nebraska Department of Motor Vehicles Identification/Address/Social Security Verification list.

Your ID card will be mailed to you after five-to-seven business days. ID card fees vary depending on the length of time before the card expires.

Need for the Document
