ProcedureEdit

  1. You must personally submit your request to obtain a Life Certificate.
  2. Go to the Civil Service Office and do not forget to bring your ID card.
  3. In cases where the applicant is unable to move for medical reasons, the certificate may be issued to an authorized representative by presenting the applicant's identification together with his/her identification, and the applicant's medical certificate.
