ProcedureEdit
- You must personally submit your request to obtain a Life Certificate.
- Go to the Civil Service Office and do not forget to bring your ID card.
- In cases where the applicant is unable to move for medical reasons, the certificate may be issued to an authorized representative by presenting the applicant's identification together with his/her identification, and the applicant's medical certificate.
