- In the event of loss or theft, you will need to report the incident to the police and obtain a certificate from the police.
- Go to the Civil Service Office submit your request to obtain a duplicate Family Record Book and do not forget to bring your ID card.
- Pay for the booklet.
- If you would like to have the duplicate Family Record Book sent to you, get in touch with the Civil Service Office (use the contact information provided below).
