Procedure

Apply In Person

1. To amend or modify a Birth, Marriage, or Death Certificate, the applicant should make a written application to the Magistrate of the district in which the entry of the document was registered for an order authorizing the Registrar of Civil Status to amend an entry.Please view the Civil Status Office contact details through this Contact Details
2. The applicant should obtain the application form for rectification from any Civil Status Office or download the form directly through this Form For Rectification.
3. Read and complete the amendment form correctly, and then make a copy of the civil status document to be amended and the applicant’s National Identification Card (NIC) and enclose then with the completed application form in a single file.
4. Then submit the application file to the Magistrate of the district in which the entry of the document was registered.
5. The magistrate will assess the application and refer the application to the Minister Public for a conclusion of the amendment application.
6. The Minister Public will deliberate on the amendment application and make a conclusive decision regarding the application.
7. The Minister Public will then for conditions of the amendment application to the Magistrate and upon receipt of those conditions, the Magistrate will either decide to grant or refuse the amendment of the civil status document.
8. If the application is approved for amendment, the registrar of the civil status documents will modify the document as required and update the civil status registry.
9. If the application is not approved, the applicant will be informed in writing and he/she is dissatisfied with the decision of the Magistrate, the applicant may within six weeks from the date of the decision, move to the Supreme Court for a reconsideration of the application.
