Procedure

Apply In Person

1. To apply for the Scaffold Contractor’s Permit, you should make your application to the Ministry of Labour, Human Resource Development and Training. Please view the ministry contact details through this link contact details.
2. Visit the ministry’s office and obtain the application form from the reception or download the form through the ministry’s website via this link Downloadable forms (Application for Scaffold Contractor's Permit). You can also download the form directly via this link Application form
3. Read and complete the application form accordingly and then prepare all the required supporting documents as mentioned in the “Required Documents” section below and attach the documents to the application form.
4. Go to the ministry’s office and pay the non-refundable fee at the cashier and obtain the payment receipt.
5. Then submit the complete application along with the payment receipt to the office of the Permanent Secretary.
6. The Permanent Secretary will review the application and if it is complete as required, the Permanent Secretary will refer the application to the Scaffold Contractor’s Permit Committee for further assessment.
7. The committee will evaluate the application and conduct a necessary investigation on the application and/or consult with the relevant bodies regarding the application or the committee may request additional information and documentation to support your application.
8. Upon the evaluation of the application, within 28 days of receiving the referral from the Permanent Secretary, the committee will submit its recommendation to the secretary as to whether the permit may be issued to you or not.
9. The Permanent Secretary will, within 7 days of the receipt of the recommendations of the Committee, issue or refuse to issue the permit to you.
10. If the permit has been approved for issuance, you will be informed through the proper channel and required to pay the permits fee of Rs.10,000.
11. Upon payment of the fee, the permit will be issued to you with conditions set out by the Permanent Secretary.
12. If the Permanent Secretary refuses to issue a permit, you will be informed in writing the grounds for such refusal. Note that this notice will be delivered to you personally or through your registered postal address.

• Complete application form.
• Certificate of incorporation of the company or the National Identity Card if the employer is an individual.
• The Business Registration Card.
• Certificate of registration of the employer with the Ministry of Social Security, National Solidarity and Environment, and Sustainable Development.
• Letter of employment submitted by an employer as regard Scaffold Supervisor.
• Contract of employment of Scaffold Supervisor, wherever required.
• Letter of authorization from the main employer, wherever Scaffold Supervisor is a full-time employee working as part-time Scaffold Supervisor with another employer.
• The National Certificate Level 4 in Scaffolding of the Scaffold Supervisor.
• Application fee payment receipt.
• Any other documents requested by the permanent secretary.

Instructions

• Any person aggrieved by the decision of the Permanent Secretary, may appeal against that decision to the Minister.
• The appeal shall be made in writing within 14 days of receipt of the notice and shall include the grounds on which it is made.
• The Minister shall dispose of the appeal within 21 days of its receipt and inform the person of his decision in writing. The decision of the Minister shall be final.

• Name and status of representative, if any, authorized to action behalf of the applicant.

• Name/ID number/contact details of scaffold supervisor.
• Declaration by the applicant.

Need for the Document

• Construction Company that wants to erect, re-position, alter, maintain, repair or dismantle scaffolds in a workplace are required to obtain a Scaffold Contractor Permit from the ministry of labor.
