Procedure

Apply In Person

1. To apply for a duplicate motor vehicle license due to loss, you should make your application to the National Land Transport Authority (NLTA). Please view the NLTA contact information through this link Contact details.
2. You should first report to the police station at your location the loss of the motor vehicle license and obtain a report from the police certifying that you have reported the loss of your motor vehicle license disc.
3. Then prepare the required supporting documents and attach them to the police report in a single file.
4. Visit the NLTA office and make your claim to the NLTA official at the service desk.
5. The official will then issue you with the necessary documentation to complete and then you will pay the application fee at the cashier section.
6. Once you have paid, submit the required documents including the payment receipt to the official at the service desk.
7. The official will check your application and confirm if you have provided all the required documentation.
8. After the verification, you will be issued with the acknowledgment receipt of the application and it will be submitted to the relevant official for processing.
9. The authority will process the duplicate motor vehicle license and issue it to you. Note that in the case of a company, a letter from the company should also be produced authorizing you to collect the duplicate license.
