Procedure

1. To apply for a Disable free Bus Pass card, you should make your application to the Disability Empowerment Unit under the Ministry of Social Integration, Social Security, and National Solidarity ( Social Security Division). 2. Visit the Ministry of Social Integration, Social Security, and National Solidarity ( Social Security Division) and go to the Disability Empowerment Unit to obtain the application form in person or download the form electronically through this link Application form.
3. Complete the form accordingly and in the case of the electronic form, upon completing the form click on the “ Print Form” button to print the form and then sign it personally.
4. Obtain all the required supporting documents as mentioned in the “Required Documents” section below and attach the documents to the completed form.
5. Visit the Ministry of Social Integration, Social Security, and National Solidarity ( Social Security Division) and submit your application to the serving officer at the Disability Empowerment Unit.
6. The official will verify your application and confirm if all the documentation for the application is provided as required and you will be provided with acknowledgment receipt of your application.
7. The application will be further assessed by the Disability Empowerment Unit and if it is determined that you meet the required merits for the free bus pass, then the application will be transmitted to the National Land Transport Authority (NLTA) for processing.
8. Upon the NLTA processing the card, they will send the card to the Ministry of Social Integration, Social Security, and National Solidarity who will then forward the card to your local social security office ready for collection.
9. The local social security office will contact you through your contact details and inform you to visit the office for the card collection.
10. Visit the office and collect the card in person or you can send a relative with an authorization letter for the collection of the card on your behalf.
