Procedure

1. Go to your local RMV Service Office.
2. Completely filled a Class D/M License and ID Card Application.
3. Submit the application with the $25.00 fee and appropriate documentation , as payment in person to any RMV full service office
4. Get hold of your temporary photo imaging Liquor ID.A new photo imaging Liquor ID will be delivered through your mail.

• At least 21 years old
• No Massachusettes valid license

Massachusetts Liquor ID $25.00

Validity

It is only five (5) years from the date it was issued
