You may directly go to the registry counter with as much as information and cash or check or money order payable to "The Commonwealth of Massachusetts"

  • The Registry counter is open to the public during the following hours:
  • The cost for each certified copy of a record is $18.00 at the Registry counter.
  • Orders placed at the Registry counter must include subjects name, date of event and place of event.
  • Genealogical research services are not available at the Registry counter.
  • The Registry is also open to the public in our research area.
  • For general information about research at the Registry of Vital Records and Statistics, call (617) 740-2600.
By Mail
  • Provide the information requested on the Mail Order Form and mail your request to:

  • Make your check or money order payable to "The Commonwealth of Massachusetts".
  • Enclose a business-size self-addressed envelope. The cost of each record includes a ten-year search if the exact date or place of event is not known.
  • If faster service is needed, you will need to order your certificate by one of the following method
    • Regular Mail - The cost for each record ordered by mail is $28.00. Orders are normally processed within 20-30 business days of receipt.
    • Expedited Mail - The cost for expedited mail service is $37.00 for each certified copy. Address your envelope to the attention of "Expedited Mail Service." Your order will be processed within seven to ten business days of receipt. (Research is not available via expedited mail service.)

  • The Massachusetts Registry of Vital Records and Statistics (Massachusetts Vital Records) issues certified copies of Massachusetts death certificates for events which occurred within the state of Massachusetts.
  • Death Certificates are available for events that occurred anywhere in the State of Massachusetts from 1921 to present.
  • The cost for each certified copy of a record is $18.00 at the Registry counter.
  • Orders placed at the Registry counter must include subjects name, date of event and place of event.
  • Genealogical research services are not available at the Registry counter.
  • The Registry is also open to the public in our research area.
  • For general information about research at the Registry of Vital Records and Statistics, call (617) 740-2600.
