- Here's how you can apply for an occupational permit / health certificate in the city of Manila. Right now, the easiest way to apply for a permit/certificate is online. You may apply using the Go Manila website or app (available for download on the app store)
- Request for a permit/certificate may be submitted in two ways. You may send your request through the official website for services of the City Government of Manila. Here’s a link – City of Manila Website – Apply Online. Or, you can also send your request by using the Go Manila App. On the website you may click the “Download Go Manila App” button to download the app or you can also search for it on the app store. You may send your request either way. The application process is the same in both cases. First time users/applicants will be asked to create an account before applying.
- After creating an account, open the site/app again and log in to your account. First time users will be asked to complete their profile information and upload a profile picture.
- After updating your profile, click the "Select Transaction" button and select “Occupational Permit / Health Certificate.”
- Complete the online form and set up a date and time for your appointment.
- Pay the fees.
- Go to the Manila Health Department's office on your appointment date and bring all the required documents.
- Your application will be processed. You will need to undergo and pass a series of tests conducted by the Department before a permit or certificate is issued to you.

- Police clearance issued by the Manila Police District (MPD)
- Cedula for the current year
- 1 government issued VALID I.D.
- 1 pc. 1 x 1 I.D. Picture
- For minor applicants:
- Original Parental Consent with Photocopy of Parent’s Valid I.D.
- Original Affidavit of Guardianship with Guardian’s Valid I.D.
- Official receipt for the Mayor’s Permit Fee
