Procedure

1. Fill in a declaration form (VEH39) stating the reason why the license is being reissued and submit it showing proof of identification.
2. Pay the fee for a new license photo card which is 25.65.

Need for the Document

An application for the issue of a replacement/re-issue of the driving license may be made with the Authority, if a valid driving license has been lost, stolen or damaged. The replacement driving license shall have the same effect, including the expiry date, as the original driving license.
