Malta - Apply for Special Identity Card and EU Disability Card (SID - EU Card)

In order to help persons with disability the Commission for the Rights of Persons with Disability (CRPD) offers a Special ID (SID-EU) Card. Persons with disabilities traveling to other participating EU Member States can show their card as proof that they have a certified disability status in their home Member State.
A holder of the SID-EU Card may apply for a number of services aimed for persons with disability within Government departments or private establishments.
Here's how you can obtain a Special Identity Card and EU Disability Card (SID - EU Card).
- Application is processed on line.

- Simply provide all the required information.
- Upload a scanned copy of your passport photo. The photo you will be uploading will be the photo printed on your EU Disability Card (provided that you are eligible for one), make sure that the photo is of good quality.
- You will also need to your medical certificate with your special ID number. If you do not have one download these form English, Maltese and have your doctor complete it. You must then scan the completed document and upload it when submitting your application.
- It will take about 10 days for your application to be processed.

- Completed application
- Scanned passport sized photo (approx. 45mm by 35mm). Light background showing the full frontal face, with no hats, helmets or sunglasses. The photo you upload will be printed on your EU Disability Card (provided that you are eligible for one), make sure that the photo is of good quality.
- Medical Certificate. If you do not have one download these form English, Maltese and have your doctor complete it. You must then scan the completed document and upload it when submitting your application.

EligibilityEdit
- Persons with disability as defined by the Social Security Act (Cap. 318.) - Act For social security
- Persons with permanent hearing impairment to a degree of not less than 70 decibels or who use a hearing aid regularly
- Persons who have permanent total paralysis or serious permanent total malfunctioning of one limb
- Persons who have severe psychological problems that cause disability

ValidityEdit
Normally a SID-EU Card is issued with a validity period of 10 years for adults. For children, cards are issued with shorter validity.
- You must renew the SID-EU Card at least 2 weeks before expiry date
- If the Card is lost, you will need to make a declaration on a form issued by CRPD for a new card to be issued
- The SID-EU Card is a legal document and thus, in the case of the owners death, the SID-EU Card must immediately be returned to CRPD

Need for the DocumentEdit
Persons with disabilities traveling to other participating EU Member States can show their card as proof that they have a certified disability status in their home Member State.
A person in possession of the SID-EU Card is entitled to benefits such as:
- Free ferry trips by Gozo Channel
- Free transport service to and from state hospitals (by appointment)
- Priority for certain services provided
- Free entrance or reduced prices for organised activities
EU Disability Cards holders will be able to use their card in any of the eight participating countries to access certain benefits. Benefits in each of the participant countries are different. People having an EU Disability Card issued by CRPD will be able to access the benefits specifically provided for EU Disability Card holders within the participating country they are traveling to, including the benefits provided in their home country (Malta).
Likewise, EU disability card holders from any of the 7 participating countries can access the specific benefits provided for EU Disability Card holders when visiting Malta.
