Apply In-Person

  1. The declarant/applicant should visit the reception office of the referral health center in his/her municipality and submit the burial permit and the registration number issued by the burial permit office.
  2. The reception office at the referral health center draws up (section 2) of the declaration of death.
  3. In principle, the referral health center administratively transmits (section 2) to the town hall of the declarant's residence, but in practice, the referral health center gives the said document to the declarant.
  4. Pay the certificate fee depending to the number of copies you wish to obtain, upon which you will be issued with an acknowledgment slip.
  5. The declarant/applicant should then head the town hall of your district, death registration department with the supporting documents (listed below under required document section) and submit them to be verified by the registrar.
  6. The secretary registers the documents, draws up the death certificate, which is then submitted to the mayor office to be signed by the mayor office.
  7. The certificate will be ready after one to three days of processing, revisit the town hall death registration department with your ID card and acknowledgment slip and calm the certificate (s).

Other uses of the Document/CertificateEdit

The death certificate is important for three basic reasons:

  • Legal Reasons: The death certificate is a permanent legal record of the fact of death. Therefore, it is a legal requirement. It provides important information about: the decedent, the cause of death, and final disposition. This information is used in the application for insurance benefits, settlement of pension claims, and transfer of title of real and personal property. The certificate is prima facie evidence of the fact of death and, therefore, can be introduced in court as evidence when a question about the death arises.
  • Personal Reasons: The death certificate in many cases provides family members with closure, peace of mind, and documentation of the cause of death. It also provides peace of mind by facilitating efficient processing of needed benefits as those described above.
  • Vital Statistics Reasons: The death certificate is the source for state and national mortality statistics. It is needed for a variety of medical and health-related research efforts. It is used to determine which medical conditions receive research and development funding, to set public health goals and policies, and to measure health status at local, state, national, and international levels. This data is valuable as a research tool and by influencing research funding.
