Procedure

1. Submit the request letter with the necessary documents to counter of Male Municipality Aabaadhee behey section.
2. If the applied name is not approved collect Nan faas kurumah dhookuraa slip from the counter. Fill the form and submit it to the counter.
3. Make the payment and obtain the payment slip. (The date and time at which to collect the name change birth certificate will be mentioned in this receipt.)
4. Go to Male Municipality on the date and time specified in the payment slip and submit it to the counter and obtain Birth Certificate with the changed name.

• Request letter (the letter must state the new name to be called, reason why you want to change the name of the child. The letter should be on the name of the childs father or mother or the guardian of the child)
• Childs birth certificate (original)
• Childs birth registry (original)
• Childs house registry (original)
• Rf 1 revenue stamp

Eligibility

The birth certificate for the child must be issued from Male Municipality.
