Procedure

1. Submit the request letter with the necessary documents to counter of Male Municipality Ge Aabadheeaai behey section.
2. Collect the Ufandhuvahuge certificate gai vaa nan badhalu kurumah edhey meehunge mauloomaathu form from the counter and fill the form by relevant government organizations and submit it to Male Municipality.
3. Make the payment and obtain the payment slip. (The time and date to collect the name changed birth certificate will be stated in the payment slip)
4. Go to Male Municipality on the date and time specified in the payment slip and submit it to the counter and obtain the Birth Certificate with the changed name.

• Birth certificate (original)
• If registered in a Male house, registry of that house (original)
• If registered in Dhafthar, Dhafthar Certificate (original)
• If the birth certificate is issued from an islands office, that birth certificate (original)
• National Identity Card (original)
• Rf 1 revenue stamp
