Procedure

• Visit the concerned authority.
• Fill the application form (Application Form JPN.KP01) with the complete information. The application form can be obtained from the NRD counter.
• Submit all the necessary documents.
• Pay the necessary fees.

Applicant's Documents

• Original Birth Certificate or Adoption Certificate or Citizenship Certificate or confirmation of citizenship status or Citizenship Form or Appendix A (JAKOA)
• Original passport if the applicant is a non-resident

Sponsor's Documents

• Original identity card
• Original document confirming guardianship from the Department of Social Welfare (for guardian who is not a relative of the applicant)
• Original passports from their country of origin (non-citizen sponsors)

Eligibility

• Must have attained 12 years of age but is below 16.
• The sponsor must be present, who is either the
  • Parent or
  • Guardian (the guardian could be the grandfather, grandmother, aunt, uncle, elder sister or brother who is over 21).

Fees

• The fee for this service is waived for children 12 years of age to 12 years and 30 days.
• If failed to apply withing 30 days, the application will incur processing fee.

Validity

Explain the time until which the certificate/document is valid.
