- Visit the concerned authority personally.
- Fill the application form with the complete information.
- Submit the necessary documents. It must be submitted by the parent or guardian.
- Pay the prescribed fee.

Registrant's Documents
- Original and copy of the Identity card
- Original and copy of the Birth Certificate, latest extract of Birth Certificate or Adoption Certificate (translated into Malay or English by the court/consular office/public translator)
- Original and copy of the Entry permit or passport or other travel document
- 3 recent passport-sized photographs with a blue background
- Original and copy of the Documentary evidence of schooling
Applicant's or Guardian's Documents
- Original and copy of the Identity card
Documents of parents and siblings
- Original and copy of the Birth Certificate or latest extract of Birth Certificate
- Original and copy of the Identity card
- Original and copy of the Citizenship Certificate
- Original and copy of the Marriage document
All copies of supporting documents should be made on A4 paper

EligibilityEdit
- For Malaysian citizenship by a person under the age of twenty-one whose parent or parents are (or were at the time of death) citizens.
- The person to be registered must be below the age of 21 on the day of application.
FeesEdit
- Application fee - RM 10.00
- Fee for the issuance of Citizenship Certificate(if approved) - RM 100.00
ValidityEdit
Explain the time until which the certificate/document is valid. e.g. Birth Certificate Valid Forever
Documents to UseEdit
- Form B (which can be obtained from the Putrajaya NRD headquarters or state NRD headquarters/branches)
- Appendix A.15 (2), which is a dual purpose form for declaration of acceptance of and application for citizenship under Article 15(2) of the Federal Constitution, must be completed and submitted to a consulate abroad within one (1) year of the date of birth
