  • Visit the concerned authority personally in accordance with Rule 28 of the Citizenship Rules 1964 (Amendment 1996).
  • Fill the application form (Form D) with complete information.
  • Submit the necessary documents.
  • Pay the prescribed fee.

Registrant's Documents

  • Original and copy of the Birth Certificate or latest extract of Birth Certificate
  • Original and copy of the Passport or entry permit
  • 3 recent passport-sized photographs with a blue background

Parent's Documents

  • Original and copy of the Birth Certificate or latest extract of Birth Certificate
  • Original and copy of the Identity card
  • Original and copy of the Passport
  • Original and copy of the Citizenship Certificate
  • Original and copy of the Marriage document

All copies of supporting documents should be made on A4 paper

  • Fee for the issuance of certificate by the consulate if approved - 10 RM
  • Fee for the issuance of certificate by the Registrar-General of Citizenship with the approval of the Minister of Home Affairs - 30 RM

Payment with postal order is accepted in multiples of 10 RM

InstructionsEdit

  • All applications for citizenship matters require the use of original forms which may be obtained from the Putrajaya NRD headquarters or state NRD headquarters/branches. Application forms for registration of birth in a foreign country and renunciation of citizenship may be obtained from consulates abroad.
  • Where a child is born before the parents marriage is registered, the child's status follows that of the mother as stated in Section 17, Part III of the Second Schedule of the Federal Constitution.
